platelet transfusion for ICH?

January Annals review by Martin & Conlon:

Does platelet transfusion improve outcomes in patients with spontaneous or traumatic intracerebral hemorrhage?

a.k.a. should we give ICH patients who take antiplatelet drugs (e.g. aspirin, clopidogrel) platelet transfusions?

 

BOTTOM LINE (HIGHLIGHTS):

no studies showed mortality benefit or improved functional outcome with platelet transfusion

unfortunately:

  • all these studies were small and retrospective
  • no standardized amount or timing of transfusions

ongoing study out there that might help clarify

no compelling data currently supporting the use of platelet transfusion in the management of patients with spontaneous or traumatic who are receiving antiplatelet medications.”

“It would be within the standard of care to withhold platelet transfusion…”
