tip of the day: vertical shear pelvic injury

VERTICAL SHEAR INJURY Refresher:

hemipelvis is displaced superiorly or posteriorly due to a longitudinal force 

Anterior pelvic ring injuries can include pubic symphysis disruption or rami fractures.

Posterior pelvic ring injuries occur most commonly through the SI joint, also fx’s of iliac wing or sacrum.

 

TIP OF THE DAY:

tape the feet together (in internal rotation, if feasible)

 

BASIC IDEA:

quick temporizing measure (obviously not a permanent fix), to reapproximate the pelvic ring, reduce the potential space for bleeding, stablize fx fragments, etc.

 

BOTTOM LINE:

vertical shear pelvic injury, tape the feet together.
