pyridoxine for INH toxicity

quick dosing reminder:

pyridoxine (vitamin B6) is used to treat INH toxicity (e.g. part of the differential for seizures)

 

WHAT’S THE TREATMENT DOSE?

equivalent to the suspected max amount of isoniazid (INH) ingested — gram for gram

if unknown ingestion, go empiric:

  • 5 grams pyridoxine IV over 5-10 min (for the avg adult)
  • 70 mg/kg first dose (peds/etc). 
    • for example: avg 70kg person * 70mg/kg = 4900mg ~= 5 grams pyridoxine

repeat dosing may be needed for persistent seizures.

 

WHAT’S THAT DOSE AGAIN?

gram-for-gram of INH ingested

5 gm (adult size), OR

70 mg/kg (peds/everyone)
